    What is Remote Patient Monitoring (RPM)?

    The focus of health care continues to shift from reactive acute care to proactive patient-centered care. Remote patient monitoring (RPM) is becoming an important component of care that contributes to better patient outcomes and higher satisfaction. Given that chronic illness accounts for 95% of all Medicare spending, there is significant opportunity to reduce healthcare costs through improved care management and coordination.

    While the goal of Medicare’s RPM programs are to encourage physicians to proactively manage patient health, as opposed to reactively treating disease and illness, CMS recognizes that chronic management is complex and requires significant planning and coordination. The introduction of the new billing codes is intended to encourage physicians and other healthcare providers to invest in RPM and care management programs (such as Principle Care Management and Chronic Care Management) and to compensate those providers for the time spent between appointments helping patients to stay on track with their treatments and achieve better health.

    Do you offer Electronic Health Record (EHR) integration?

    Yes, we offer deeper integration with popular electronic health records. However, our simple, low-risk, and no start-up cost option allows you to get started on day one offering options to copy and pasting reports into patients’ charts – no integration required.

    How do I get started?

    After you contact us at sales@patientpremier.com. We will have you create an account, convert it to a physician account, and have you sign a master services agreement.

    From there, our customer support team walks you through the onboarding and workflow process for both patients and physicians getting you up to speed quickly.

    How is Pain Scored reimbursed?

    Medicare and national payers reimburse for remote monitoring and care coordination using CPT codes. Providers may bill for CPT Codes each month as long as the individual billing requirements for each code are separately met at the end of each month.

    What assessments are provided?

    Pain Scored is intended to be turnkey. Our platform comes standard with four core assessments for depression, cognition, risk stratification, and pain -- optimized for compliance, liability, revenue and outcomes. You may easily design your own assessments for use in Pain Scored.

    How much time will Pain Scored add to my workflow?

    Remote patient monitoring allows for previously non-reimbursable care coordination to be reimbursable. Therefore, time spent on the platform and off the platform performing care coordination is now time well spent. Our workflows are made by healthcare providers for healthcare providers to be optimized for compliance, liability, revenue and outcomes – as well as provider time!
